B超检测妊娠晚期子宫下段厚度准确性的探讨

目的:探讨B超检测妊娠晚期子宫下段厚度的准确性。方法:按照是否有手术史将139例孕妇分为两组,其中88例至少有一次剖宫产史,51例无子宫手术史。全部孕妇均进行剖宫产术前B超及术中子宫下段厚度测量,对比不同组别间B超与术中测量的子宫下段厚度,以及同组间术前B超及术中测量的差别。结果:术前1周内B超测量子宫下段厚度与术中测量无统计学差异(P>0.05)。无手术史组子宫下段厚度大于有剖宫产史组(P<0.05)。3例瘢痕子宫孕妇术前B超未发现异常,术中发现2例子宫下段肌层不全裂开,1例肌层完全裂开。结论:分娩前有剖宫产史者子宫下段厚度薄于无剖宫产史者。分娩前期B超检测情况基本可以反映子宫下段的真实厚度,但并不是阴道试产的准确依据。

Study on the accuracy of B ultrasonography for detection of lower uterine segment thickness during the third trimester of pregnancy

Objective:To explore the accuracy of B ultrasonography for detection of lower uterine segment thickness during the third trimester of pregnancy.Methods:A total of 139 pregnant women were divided into two groups according to with cesarean section history or not,88 women had cesarean section history at least one time,and 51 women had no uterine surgery history.All the cases underwent preoperative ultrasonic measurement and intraoperative measurement of lower uterine segment thickness,then the intragroup and intergroup results were compared.Results:There was no significant difference in the lower uterine segment thickness at one week before operation between the two groups(P>0.05).The thickness in non-cesarean section history group was thicker than that in cesarean section history group(P<0.05).There was no abnormality in preoperative ultrasonography of 3 cases with scar uterus,two cases with incomplete rupture of lower uterine segment and one case with complete rupture of lower uterine segment were found.Conclusion:The lower uterine segment thickness of cases with cesarean section history was thinner than that of cases without cesarean section history,prenatal ultrasonography can reflect the real thickness,but it is not the accurate basis of vaginal trial produce.
