单脐动脉合并胎儿先天性结构畸形的产前超声诊断及分析

目的:评价产前超声诊断单脐动脉及合并胎儿先天性结构畸形的应用价值。方法:回顾性分析接受产前筛查的142例单脐动脉及72例单脐动脉合并胎儿先天性结构畸形的超声特点,分析单脐动脉合并畸形类型及胎儿结局。结果:142例单脐动脉中合并胎儿先天性结构畸形72例,发生率为50.70%(72/142);单脐动脉合并多发畸形较单发畸形多见,其中伴发1种畸形26例占36.11%(26/72);合并2种及2种以上畸形46例占63.89%(46/72);合并畸形中居前三位的分别为心血管系统、泌尿系统、中枢神经系统。结论:产前超声检查可较为准确、及时的发现单脐动脉,并能进一步检查胎儿有无合并先天性结构畸形,从而有效指导临床产科处理,有利于优生优育。

Prenatal ultrasonic diagnosis and analysis on single umbilical artery combined with fetal congenital structural abnormality

Objective:To evaluate the application value of prenatal ultrasound in diagnosis of single umbilical artery combined with fetal congenital structural abnormality.Methods:The ultrasonic characteristics of 142 cases with single umbilical artery and 72 cases with single umbilical artery combined with fetal congenital structural abnormality who were found in prenatal screening were analyzed retrospectively,the types of single umbilical artery combined with fetal congenital structural abnormality and fetal outcome were analyzed.Results:Among 142 cases with single umbilical artery,72 cases were found with single umbilical artery combined with fetal congenital structural abnormality,the incidence was 50.70%(72/142);single umbilical artery combined with multiple abnormality was more common than single umbilical artery combined with single abnormality,26 cases were found with single umbilical artery combined with one kind of abnormality,accounting for 36.11%(26/72);46 cases were found with single umbilical artery combined with two or more than two kinds of abnormality,accounting for 63.89%(46/72);the top three abnormalities were cardiovascular system,urinary system,and central nervous system.Conclusion:Prenatal ultrasonic examination can find single umbilical artery accurately and timely,which can further examinate whether the fetuses are combined with congenital structural abnormality,and it can direct clinical obstetric treatment and be helpful to eugenics.
