慢阻肺所致肺心病急性发作期肺动脉高压治疗的系统研究

摘    要:本研究系统观察了92例慢性肺心病急性发作期患者,使用4类6种血管扩张药物,其中包括有硝普钠,氨吡酮,甲氰吡酮,异搏定,硫氮酮,开搏通;吸入低浓度一氧化氮;施行机械通气。各自比较使用前后其血流动力学及氧动力学变化。观察结果表明,上述药物及使用不同方法,均可有效地降低肺动脉压力、阻力,有良好的急性血流动力学效应及氧动力学效应,本研究分析了上述各种降压药物及方法的作用特点和相应临床应用指征,对选择临床治疗方法有指导意义。

Abstract:The paper reported the comparative effects of six vasodilators,including Sodium Nitraprusside, Amrinone, Milrinone, Verapamil Diltiazem and Captopril administration, inhalation of low concentration nitric oxide (NO) as Well as mechanical ventilation in the treatment of 92 cases of Patients with chronic cor pulmonale at acute exacerbation stage. Hemodynamic and Oxygen- dynamic changes were observed prior to and after each treatment.They all showed effective result in lowering pulmonary hypertension, pulmonary vascular resistance and exerted other favorable hemodynamic effects. The characteristics and clinical indications of each vasodilator were analyzed,Which might significant for the choice of vasodilaor in clinical practice.
