梗阻性黄疸的影像学诊断及选择

摘    要:目的 探讨US,CT,ERCP,MRCP对梗阻性黄疸的诊断价值。方法 109例病人经手术或病理证实为梗阻性黄疸作为对象,对其US,CT,ERCP,MRCP检查结果做回顾性分析。结果 US,CT,ERCP,和MRCP的总诊断率分别为6880%(75/109),7615%(83/109),960%(48/50),100%(15/15)。不同的诊断方法对不同的疾病敏感度不同。联合应用不同的诊断方法能改善诊断的敏感度。结论 影像学诊断对梗阻性黄疸的定性及定位起着决定性的作用,US应当做为首选方法,联合应用不同的影像学技术可以显著提高诊断率。

Abstract:Objective To evaluate the efficiency of imagine diagnosis of obstructive jaundice by US, CT, ERCP, and MRCP. Methods Total 109 cases were studied, who were diagnosed pathologily or postopratively. The date from all the cases, including US, CT, ERCP, and MRCP were analyzed respectively. Results The total correct diagnosis rates were 68 80%(75/109),76.15%(83/109),96.0%(48/50), and 100%(15/15) for US, CT, ERCP, and MRCP respectively. Different technology had different sensitivity for different obstructive jaundice. Combined means can improve the diagnostic efficiency greatly. Conclusion Based on the study and similar research, it is confirmed that imagine technology play a decisive role in the quantity and position diagnosis of obstructive jaundice. One had better use US as the first choice for the diagnosis, because of its simplicity, economy, safety and reliability, and should use comprehensive method to clarify the diagnosis when the situation is confused.
