损伤控制性外科在合并腹部创伤的严重多发伤中的临床应用

目的通过总结大量临床的病例,对损伤控制性外科(Damage Control Surgery,DCS)技术在严重多发伤合并腹部创伤的临床应用进行分析。方法回顾性分析广东省某3所医院2007年1月²013年1月收治的226例严重多发伤合并腹部创伤患者的临床资料,按照术前ISS评分标准进行分组,A组为162013年1月收治的226例严重多发伤合并腹部创伤患者的临床资料,按照术前ISS评分标准进行分组,A组为16²4分,B组为2524分,B组为25³4分,C组为≥35分。比较各组的年龄、ISS评分、损伤部位、复苏时间、手术部位、确定性手术时间、住院时间、总输血量、并发症发生率、病死率。结果三组年龄和损伤部位差异无统计学意义(p>0.05),手术部位、确定性手术时间、住院时间、输血量、并发症及病死率比较差异有统计学意义(p<0.05)。结论采用DCS治疗严重多发伤合并腹部创伤的患者效果良好,值得推广。

APPLICATION OF DAMAGE CONTROL SURGERY IN THE SEVERE MULTIPLE INJURIES COMBINED ABDOMINAL TRAUMA

Objective To summarize large number of clinical cases to analyze clinical application of damage control surgery （DCS） technology in severe multiple injuries combined with abdominal trauma. Methods We retrospectively analyzed the clinical data of three hospitals in Guangdong Province from January 2007 to January 2013 admitted 226 cases severe muhiple trauma combined with abdominal trauma patients, whom were subdivided into groups according to the preoperative ISS assessment criteria： group A of 16 - 24 scores, group B of 25 -34 scores, group C≥35 scores. Compare the ages, ISS score, injured position, recovery time, the surgical site, the implementation of definitive surgery, hospitalization time, the total amount of blood transfusion, complications and mortality between three groups. Results The ages and injured position have not statistical significant difference between three groups （p 〉 0. 05 ）, the differences of surgical site, definitive surgery, hospitalization time, blood transfusion, complications and mortality were of statistical significances （p 〈 0. 05 ）. Conclusion The effect DCS in treating severe multiple injuries combined with abdominal trauma is good, worthy of promotion.
