螺旋CT平扫早期诊断肠和肠系膜钝性伤的临床价值

摘    要:目的 探讨螺旋CT平扫对肠及肠系膜钝性伤的早期诊断价值.方法 回顾性分析32例经手术证实的急性肠及肠系膜钝性伤的CT表现和临床资料,所有病例均行CT平扫及增强检查,总结其CT征象的临床意义,并对CT平扫及增强检查结果进行对比分析.结果 螺旋CT平扫及CT增强在腹腔积液或血肿、腹腔游离气体、肠系膜渗出及血肿、肠壁肿胀、肠管破裂、肠管扩张伴积液方面,诊断率无显著性差异.结论 螺旋CT平扫对早期诊断急性肠和肠系膜钝性伤有较大临床价值,增强检查可作为CT平扫的重要补充.

Abstract:Objective To discuss the clinical value of spiral CT plain scan in the early diagnosis of bowel and mesenteric blunt trauma. Methods Retrospectively analysis was made in the CT manifestation and clinical data of 32 cases who were diagnosed as acute bowel and mesenteric blunt trauma by operation. All the cases received the CT plain scan and contrast-enhanced examination. The clinical significance of the features was summarized, and the results of the examinations were comparatively analyzed. Results There was no difference in the diagnosis rates of seroperitoneum or hematoma, peritoneal cavity free air, mesenteric exudation and hematoma, bowel wall swelling,intestinal canal rupture, and intestinal canal enlargement complicated with fluid between spiral CT plain scan and contrast-enhanced CT. Conclusion Spiral CT plain scan has great significance in the early diagnosis of acute bowel and mesenteric blunt trauma. Contrast-enhanced CT may used as an important complementary technique.
