有无自杀意念男性海洛因依赖者心理状况的比较

目的探讨有无自杀意念男性海洛因依赖者(MHDP)心理状况的差异。方法采用自编一般情况问卷、症状自评量表(SCL-90)、多伦多述情障碍量表(TAS)、渴求问卷(HCQ)、抑郁自评量表(SDS)与焦虑自评量表(SAS)对218例康复期MHDP进行评定,有194例MHDP纳入研究。组间各评分比较采用t检验,单因素分析采用Pearson相关分析,多因素分析采用Logistic回归分析。结果有自杀意念者88例(45.4%)。有自杀意念组SDS、SAS、SCL-90、TAS、HCQ总分高于无自杀意念组(t值分别为5.94、6.92、9.03、3.33、2.32,P0.05或P0.01)。MHDP自杀意念与海洛因使用年限、戒毒次数、强制戒毒次数、吸烟年限、饮酒年限呈显著正相关(r值分别为0.346、0.197、0.230、0.720、0.333,均P0.01);SCL-90与SDS总分依次进入自杀意念的回归方程。结论有自杀意念较无自杀意念MHDP存在更为严重的心身症状、述情障碍、心理渴求与负性情绪。不良的心身健康状况及抑郁情绪是MHDP自杀意念产生的重要因素。

A comparison study on psychological characteirs tics in male heroin depe ndence patein ts with and wit hout suicide ideation

Objective To study the difference on psychological status in male heroin dependence patients（MHDP）with and without suicide ideation.Methods A total of 218 MHDP were assessed with a self-designed questionnaire,symptom checklist 90（SCL-90）,Toronto alexithymia scale（TAS）,heroin craving questionnaire（HCQ）,self-rating depression scale（SDS）and self-rating anxiety scale（SAS）,and there were 194 MHDP finished the test effectively .The scores between the two groups were compared by t test,univariate analysis were studied by Pearson correlation analysis ,multivariate analysis were studied by Logistic regression analysis.Results Compared with MHDP without suicide ideation , the patients with suicide ideation had significant higher total scores of SDS,SAS,SCL-90,TAS and HCQ（t=5.94,6.92,9.03,3.33,2.32,P＆lt;0.05 or P＆lt;0.01 ） .The suicide ideation showed significant positive correlation to the use age , number of times of detoxification ,number of times of compelling detoxification ,smoking age and drinking age in MHDP （r=0.346,0.197,0.230,0.720,0.333,all P＆lt;0.01）.The total scores of SCL-90 and SDS entered the regression equation of SCL-90 in turn.Conclusions Compared with MHDP without suicide ideation ,MHDP with suicide ideation have severe psychosomatic symptoms ,alexithymia ,craving and negative mood .The worse psychosomatic status and depressive mood are responsible for suicide ideation in MHDP .
