C反应蛋白、D-二聚体及乳酸的测定对危重症患者预后的临床价值

目的：探讨血清C反应蛋白、D-二聚体水平、乳酸对危重症患者预后的临床价值。方法：回顾性分析本院2008年1月～2008年4月急诊ICU收治住院的危重症患者30例,入ICU24h测CRP、D-二聚体、乳酸水平,并进行急性生理学与慢性健康状况评分Ⅱ及出院评定。结果：预后不良组（无改善＋死亡）CRP值为（13.8±9.6）mg/L,D-二聚体值为（12.20±5.60）mmol/L,LA为（7.1±4.5）g/L明显高于预后良好组（明显改善＋治愈）CRP值为（5.02±4.61）mg/L（P〈0.05）,D-二聚体值为（2.73±0.81）mmol/L,乳酸为（2.1±1.4）g/L。结论：血清CRP、D-dimer、LA水平可作为危重症患者评定病情严重程度及预后的重要指标。

C-reactive protein,D-dimer and the determination of lactate in critically ill patients with prognostic value

Objective：To investigate the serum C-reactive protein,D-dimer levels,lactic acid on the clinical prognosis of critically ill patients value.Methods：A retrospective analysis of our hospital in January 2008 to April 2008 admitted to the hospital emergency ICU 30 cases of critically ill patients,into the ICU 24 hour measured CRP,D-dimer,lactate levels,and the acute physiology and chronic health evaluation Ⅱ and discharge assessment.Results：The poor prognosis group（no improvement＋death） CRP values（13.8±9.6） mg/L,D-dimer value（12.20±5.60） mmol/L,LA was（7.1±4.5） g/L significantly higher than the good prognosis group（significant improvement＋cure） CRP values（5.02 ±4.61） mg/L（P〈0.05）,D-dimer values（2.73±0.81） mmol/L,lactic acid（2.1±1.4） g/L.Conclusion：Serum CRP,D-dimer,LA levels in critically ill patients can be assessed as the severity and prognosis of important indicators.
