影响高血压幕上脑出血预后的因素

目的探讨高血压幕上脑出血(ICH)患者30d预后的影响因素,为临床治疗提供依据。方法对我院2002-01～2006-12住院的309例高血压幕上ICH患者的有关指标与30d预后的关系进行单因素和多因素分析。预后据神经功能缺损评分分为好转(基本痊愈、显著进步和进步)和未好转(病情无变化、恶化和死亡)。结果Logistic多因素分析提示血肿量、中线移位、院内感染、多器官功能衰竭、消化道出血、BI评分、营养支持是影响预后的相关因素。结论积极营养支持、清除血肿、降低颅内压、防止并发症是改善预后的重要环节。

Influencing factors of the prognosis of hypertensive supratentorial intracerebral hemorrhage patients

Objective To explore the influencing factors of 30- day prognosis in 309 hypertensive supratentorial intracerebral hemorrhage(ICH) patients in order to provide theoretical basis for clinical treatment. Methods Related indexes in 309 hypertensive supratentorial ICH patients were observed. The relationship between these factors and 30- day prognosis was analyzed by univariate and multivariate analysis. According to the severity of neurological dysfunction, the prognosis was rated as good(including recovery,significant improvement and improvement), and bad(including no change, deterioration and death). Results Logistic Stepwise Regression Analysis showed nutritional support, volume of hemorrhage, median line aversion, Barthel Index, nosocomial infection, multiple organ failure, upper gastrointestinal hemorrhage influenced the prognosis of supratentorial ICH. Conclusion To supply sufficient nutritional support, to prevent the enlargement of hemorrhage, to lower intracranial pressure and to prevent complications are key to the improvement of prognosis.
