流产未必都妊娠终止--对先兆流产传统定义的刍议

目的　探讨先兆流产发生率和预后。方法　对国人随机进行孕期出血调查后给予回顾性分析。结果　 6 0 7例孕次中 ,共有 111例在孕期发生阴道流血 ,其发生率为 18 3%。在出血的 111例中 ,97例 (93% )是发生在孕 2 8周前 ,故此率亦可大致视为先兆流产率 ,即 16 4 %。有一小半先兆流产 (四成 )可以足月分娩(含少许早产 ) ,最后求出先兆流产中妊娠丢失率为 5 7 0 % ,足月分娩率为 4 3 0 % ,自然人群中的妊娠丢失率为 10 1%。结论　发生在孕 2 8周内孕期出血率 16 4 % ,可视为先兆流产发生率。孕期出血者仍有 4 3%可以足月分娩 ,所以一遇出血即认为不宜继续妊娠的做法是过分行为 ,建议将流产定义“妊娠 <2 8周而中止者”中“中止”改为“有终止症征者”。

Not all of threatened abortion need terminat pregnancy--Contrative opinion to traditional definition of threatened abortion

Objective To explore incidence of threatened abortion and prognosis.Methods We surveied pregnancy bleeding in Chinese radomly and analyzed the data retrospectively.Results Among 607 graviditas,there are 111 cases with colporrhagia duraing pregnancy,the incidency was 18.3%.In the 111 cases,97 cases(93%) occurred in the first 28 weeks of pregnancy,which meaned that the rate of threatened abortion was 16.4%.We calculated that the rate of pregnant lose was 57.0% in threatened abortion.The rate of term delivery was 43.0%.The rate of pregnant lose in natural group was 10.1%.Conclusion The rate of vaginal bleeding within the first 28 weeks of pregnancy is 16.4% which can be regarded as the incidence of threatened abortion.There are 43% pregnancy with bleeding can still term labor.So we think it is excessive action that once bleeding happened,the pregnence had to stop.And we suggest that"termiante" be changed to "have indicator of terminate" in the definition of abortion.
