青光眼滤过手术联合丝裂霉素C的应用

目的：观察丝裂霉素C（MMC）在青光眼滤过性手术中应用的临床疗效。方法：对2007年9月到2008年8月入住我院的36例各种类型青光眼患者（42眼）的抗青光眼滤过手术中应用丝裂霉素C，手术采用以穹窿为基底的结膜瓣，做4mm×5mm、厚1／2梯形巩膜瓣。观察组21眼在青光眼滤过性手术中单独一次使用丝裂霉素C（MMC）0．3mg／ml；对照组21眼除不给予MMC外，其余治疗同观察组。做术后观察、随访，统计其眼压、视力与视野的改变和滤过泡及并发症的情况。结果：观察组较对照组的眼压和滤过泡在术后观察和随访中都有显著性差异（p〈O．05）；视力与视野术前和术后随访均无显著差别（p〉0．05）；观察组未发现并发症，对照组共有3眼发生并发症。结论：青光眼滤过性手术联合MMC，可以减少滤过泡瘢痕化的形成，延长滤过泡功能，降低眼压，提高滤过性手术成功率。

A Clinical Investigation on Glaucoma Filtering Operation Combined with MMC

Objective: To evaluate the clinical effect glaucoma filtering operation combined with mitomycinMethod: The filtering operation is finished as routine with 0.4mg/mml cotton slice placed under conjunctiva and sclera flap. 36 glaucoma patients .42eyes) were divided into treatment group and control group. Treatment group 21 eyes were treated with glaucoma filtering operation combined with single-used MMC 0.3mg/ml , while control group 21 eyes without MMC. Result: The controlled ratio of intraocular pressure and filtering bleb of treatment group have significant difference compared with control group postoperatively (p<0.05); while the postoperative vision and visual field were not improved significantly in both treatment group and control group(p>0.05)in 36 cases; there were 3 cases turned out postoperative complications in control group, but no case in treatment group. Conclusion(The filtering operation combined with MMC not only decreased the scar of filtering bleb.improved the function of the filtering bleb-lowered intraocular pressure. but also promoted the successful ratio of filtering operation.
