经阴道三维超声诊断子宫内膜病变的临床应用

摘    要:目的 探讨经阴道三维超声诊断子宫内膜病变的价值。方法 对经阴道二维超声检查疑似宫腔内病变的46例已婚妇女进行经阴道三维超声检查,其结果与宫腔镜及病理相对照。结果 46例患者中子宫内膜增生16例,子宫内膜息肉12例.黏膜下子宫肌瘤9例,子宫内膜癌3例.余6例为正常子宫内膜分泌期改变;二维经阴道超声、三维经阴道超声诊断子宫内膜增生的特异性、敏感性、准确性分别为66.7%、93.7%、76.1%和90.0%、100%、93.5%;诊断内膜.息肉的特异性、敏感性、准确性分别为88.2%,58.3%、80.4%和100%、83.3%、95.7%,结论 三维经阴道超声提高了子宫内膜病变的诊断水平.与二维经阴道超声结合是诊断子宫内膜病变的首选方法。

Abstract:Objective To evaluate the diagnostic value of three dimensional transvaginal sonograpy( 3D TVS) for uterine cavity lesions.Methods Two dimensional transvaginal sonography(2D TVS) and 3D TVS were performed on 46 patients with irregular vaginal bleeding.The diagnosis were compared with the results of pathology and hysteroscopy.Results Of 46 patients,16 cases were diagnosed as endometrial hyperplasia,12 cases as endometrial polyp,9 cases as submucous myoma,3 cases as endometrial cancer, and the remaining 6 cases as normal secretive phase.To endometrial hyperplasia,the specificity,sensitivity,accuracy of 2D TVS and 3D TVS was 66.7% , 93.7% , 76.1% and 90.0% ,100%, 93.5% respectively.To endometrial polyp,the specificity,sensitivity,accuracy of 2D TVS and 3D TVS was 88.2% , 58.3% , 80.4% and 100%, 83.3% , 95.7% respectively.Conclusions 3D TVS improves the diagnosis of uterine cavity lesions,3D TVS combined with 2D TVS is the first choice for uterine cavity lesions.
