气管推移训练对颈椎前路手术的影响

摘    要:目的 查明术前气管推移训练对血压、心率、呼吸等的影响,确定最佳的累积推移时间。方法 对58例颈椎前路手术患进行气管推移训练,监测血压、心率、呼吸、吞咽动作及血氧饱和度的变化。对照组15例为颈椎前路手术而不宜行气管推移的患。结果 随着累积推移时间的增加,推移对血压、心率、吞咽动作的影响逐渐减小,累积时间达600min左右时,推移对上述指标几乎无影响。手术中训练组各项指标的变化不如对照组明显。血氧饱和度不受气管推移的影响,术中两组均无显变化。结论 气管推移训练有利于颈椎前路手术的顺利施行,训练累积时间应不少于600min。

Abstract:Objective To investigate the effect of preoperative trachea push-off on some vital signs such as blood pressure, pulse rate and respiratory rate, and determine the optimal cumulative push-off time that would best facilitate the surgical intervention. Methods 58 patients who were to undergo cervical surgery via an anterior approach received trachea push-off training, while being monitored for the changes in blood pressure, pulse and respiratory rates, swallowing and oxygen saturation of blood. 15 patients who were to have cervical surgery but not amenable to trachea push-off served as controls. Results The impact of trachea push-off lessened over time and it lev-eled off on approximating a cumulative length of time of 600 minutes. The changes in vital signs were less conspicuous in the study group than they were in controls. Oxygen saturation of blood was not sensitive to trachea push-off manoeuvre and remained unchanged in both groups. Conclu-sions Preoperative trachea push-off facilitates the manipulation of cervical surgery via an anterior approach, on condition that the cumulative push-off time is no less than 600 minutes.
