食管引流型喉罩在腹腔镜卵巢囊肿切除术中的应用

目的:探讨食管引流型喉罩与传统气管插管在腹腔镜卵巢囊肿切除术中的应用比较。方法:采用拟行腹腔镜卵巢囊肿切除术病人60例,分成PLMA组(30例)和TT组(30例),麻醉诱导后分别行食管引流型喉罩和普通气管插管控制通气。记录两组病人插管(罩)前,插管(罩)后1min,气腹前、气腹后15min,拔管(罩)前及拔管(罩)后3min血压,平均动脉压,心率,手术后恶心呕吐的次数,咽喉部不适感的例数。结果:PLMA组插喉罩后1min,拔罩后3min血压,心率,平均动脉压波动明显小于TT组(P<0.05),气腹后15min两组波动变化无明显差异,PLMA组恶心呕吐的次数和咽喉部不适感明显少于TT组,两者有显著性差异(P<0.01)。结论:食管引流型喉罩引起心血管应激反应轻,血流动力学稳定,手术后病人恶心呕吐,咽喉部不适感的发生率低。

ProSeal Laryngeal Mask airway in Laparoscopic Ovarian Cyst Resection

Objective:To investigate the ProSeal laryngeal mask versus conventional tracheal intubation in laparoscopic ovarian cyst resection application comparison.Methods:using laparoscopic ovarian cyst resection patients:60cases,divided into PLMA group(n =30)and group TT,after induction of anaesthesia were ProSeal laryngeal mask airway and endotracheal intubation and controlled ventilation in general.Records of two groups of intubation of a patient,1min after intubation,pre operation,at 15min after extubation,before and after extubation 3min blood pressure,mean arterial pressure,heart rate,nausea and vomiting after operation times,throat discomfort cases.Results : in the PLMA group of inserting laryngeal mask after 1min,pulling the cover after 3min of blood pressure,heart rate,mean arterial pressure fluctuations significantly lower than that in group TT(P<0.05),at 15min after two groups of fluctuation of no significant difference,PLMA group of nausea and vomiting frequency and throat discomfort was obviously less than group TT,there was obvious difference between the two(P<0.01).Conclusion : the ProSeal laryngeal mask airway to cause cardiovascular stress response,stable hemodynamics of patients after operation,nausea,throat discomfort occurrence rate is low.
