颈动脉内膜切除治疗暂时性脑缺血发作

本文报告颈动脉内膜切除治疗TIA45例,所有患者均有典型的TIA表现,并经血管造影证实颈动脉颅外段狭窄。全组手术无死亡。除2例术后一周内各有一次TIA,2例对侧肢体肌力减退(分别在2天和5天后恢复),1例轻度声嘶外,余均顺利恢复。作者讨论了颈动脉内膜切除在TIA防治中的作用,TIA患者颈动脉内膜切除的指征和手术时机,以及麻醉、术中监护、高位颈动脉狭窄、术后处理等问题。

Carotid endarterectomy for transient ischemic attack

Fourty-five patients with transient ischemic attack (TIA) were treated by carotid endartere ctomy. All of them had typical manifestation of TIA and stenosis of extracranial carotid artery, verified by angiography. All but five, who experienced TIA, contralateral reversible weakness and mild hoarseness respectively, had uneventful postoperative course. There was no mortality. The role of carotid endarterectomy in the prevention and treatment of TIA, the indication and rational time of endarterectomy for TIA, and some technical factors including anesthesia, intraoperative monitoring, management of stenosis of high subcranial internal carotid artery, and postoperative care, were discussed.
