冠状动脉结扎法与双极电凝法制备兔急性心肌梗死模型的对比

摘    要:目的对比冠状动脉结扎法和双极电凝法制备兔急性心肌梗死模型的差异。方法新西兰兔20只,随机分为冠状动脉结扎组和双极电凝组,各10只。冠状动脉结扎组麻醉后,胸骨正中切开,暴露心脏剪开心包,找出冠状动脉左前降支,应用5-0丝线在左冠状动脉根部下5 mm处穿线结扎,制备兔心肌梗死模型。双极电凝组麻醉后,在剑突左上第2肋骨处切开,暴露心脏剪开心包,找出冠状动脉左前降支,应用双极电凝器迅速在左冠状动脉根部下5 mm处高温烧灼,制备兔心肌梗死模型。结果冠状动脉结扎组有5只兔模型制备成功,3只手术过程中死于心室颤动,1只死于肺不张,1只术后并发感染死亡;手术共耗时10 h。双极电凝组10只兔全部成功完成模型制备;手术共耗时5 h。两组术后即可见相关导联ST段抬高,T波倒置,术后1 d ST段抬高>0.1 mV。肌酸激酶同工酶及血浆肌钙蛋白均明显升高并呈动态演变。病理学切片显微镜下观察两者梗死区均坏死明显,细胞内结构溶解消失。结论运用冠状动脉结扎法和双极电凝法均能成功制备兔心肌梗死模型,但双极电凝法具有更简便、损伤小、死亡率低、存活时间更长等优点。
