Paget病的临床与病理分析

目的探讨Paget病的临床、病理学特征、恶性程度及预后。方法收集18例Paget病患者资料,观察和分析其临床和组织病理学特征,并用免疫组化方法检测其p53、bcl-2及P-gp的表达。结果Paget病的典型皮损为湿疹样境界清楚的浸润性红斑,组织病理以表皮内有透明胞浆的Paget细胞为特征。18例Paget病均有一定程度的p53、bcl-2和P-gp的阳性表达,且均显著高于正常对照组(P<0.05)。结论Paget病为低恶性的皮肤肿瘤,有特殊的临床及组织病理特点,免疫组化有助于肿瘤的诊断和判断恶性程度及预后。首选手术治疗、长期随访。

Clinical and pathological features of Paget''''s disease

Objective: To evaluate the clinical and pathological features, severity of malignancy and the prognosis of Paget's disease (PD). Methods: Data on clinical profiles and pathological features were analyzed, and the expression of p53, bcl-2 and P-gp was measured by immunohistochemical SP techniques in 18 patients with PD. Results: Clinically typical lesions of the disease were well defined eczamatoid erythema. Histological examination showed clearing cytoplasm in the epidermis of Paget's cells. The expression of p53, bcl-2 and P-gp were higher than that in controls (P<0.05). Conclusion : PD is a skin malignant tumor with it's typical clinical and pathological features with low degree malignancy. The diagnosis, degree of malignancy and the prognosis of PD can be predicted by immunohistochemical staining. Extensive excision of the lesions with long-term follow-up is the first choice in the treatment of PD.
