胰岛素抵抗与高血压合并腔隙性脑梗死关系的研究

目的:研究胰岛素抵抗(IR)、血脂异常与高血压合并脑梗死之间的关系,以及IR与腔隙性脑梗死的关系.方法:测定67例高血压合并脑梗死患者的血脂、血糖、游离胰岛素(FIns)以及胰岛素敏感指数(ISI),并与30例健康对照者进行比较.结果:高血压合并脑梗死病人血脂、血糖、FIns、ISI与对照组比较均有极显著性差异(P＜0.01),腔隙性脑梗死组FIns明显高于非腔隙性脑梗死组(P＜0.05),腔隙性脑梗死组ISI较非腔隙性脑梗死组明显减低(P＜0.05).结论:高血压合并脑梗死存在胰岛素抵抗,胰岛素抵抗可能是独立危险因素之一,腔隙性脑梗死可能直接与IR有关.

ON THE RELATIONSHIP BETWEEN INSULIN RESISTANCE AND HYPERTENSION WITH LACUNAR CEREBRAL INFARCTION

Objective:To study the relationship between insulin resistance (IR),blood lipid abnormality and hypertension with cerebral infarction and the correlation between IR and lacunar infarction.Methods:The blood lipid,glucose and insulin were detected in the cases of hypertension with cerebral infarction and compard with those in 30 controls.Results:There were differences among blood lipid,fasting glucose,FIns(free insulin) and ISI(insulin sensitive indexes) that were compared with 30 healthy controls ( P <0 05).FIns in patients with lacunar infarction was significantly lower than that in patients with non lacunar infarction.Conclusion:IR existed in hypertension with cerebral infarction and probably was one of the independent risk factors in hypertension with cerebral infarction.Hypertension with lacunar infarction was related to IR directly.
