微型钩刀治疗腕管综合征12例报告

目的 观察微型钩刀治疗腕管综合征的疗效。方法 在掌长肌腱尺侧，距腕横纹近侧1cm作1cm长的横切口，分离到深筋膜并切开，插入槽型扩张导管，在槽的导引下插入微型钩刀，从远至近把腕横韧带完全钩割开。结果 术后4周随访，12例中10例症状完全消失，2例症状改善；术后随访1年，3例术前大鱼际肌肉明显萎缩，都有明显好转。结论 微型钩刀治疗腕管综合征是一种简单、安全有效的治疗方法。

Carpal tunnel release by tiny hook knife: A report of 12 cases

Objective To observe the outcome of carpal tunnel release by tiny ho ok knife.Methods At the unlnar of palmaris longus tendon,and1cm from proximal c arpal transverse,an incision about1cm in length was made.Following the incisi on,the subcutaneous fat was forced apart with a blunt instrument and the carpal fascia was exposed.After longitudinal incision of the carpal fascia,the space between the carpal fascia and the ligamentum carpi transversum must be exposed and widened with the various dilation conductors offerd.The knife was inserted into the dilation conductor and slowly pushed forward until the distal edge of t he carpal ligament could be hooked,and then the knife was gently pulled back.R esults Following-up of the patients4weeks postoperatively showed that the symp toms of numbness and pain disappeared in10cases,the other2case were better t han before.One year after the operation,3patients who sustained preoperative thenar muscle atrophy was greatly improved.Conclusion Carpal tunnel release by tiny hook knife is a safe,simple and effective surgical procedure.
