显微血管减压术治疗三叉神经痛疗效分析

摘    要:目的:探讨显微血管减压术治疗三叉神经痛长期随访影响预后的因素。方法:经显微血管减压术治疗三叉神经痛并获完整随访资料的187例病人,平均随访期7.38年,用时序检验及COX回归模型分析影响预后诸因素。结果:手术效果优者164例(87.7%),良者23例(12.3%)。随访期间33例有不同程度的复发,中位复发时间为6个月,临床总有效率为82.4%,因复发大多在术后2年内,单因素分析结果表明神经血管压迹与临床有效率有关,单因素及多因素分析显示静脉性压迫为复发最危险因素。结论:显微血管减压术是治疗三叉神经痛的首选方法,重视影响预后诸因素,选择治疗术式是必要的。

Abstract:To evaluate the factors that affect the the long-term prognosis of patients with trigemi-nal neuralgia treated by the microvascular decompression. Method: One hundred and eighty seven cases with trigeminal neuralgia treated by microvascular decompression, and the prognostic factors were evaluated with log-rank test and cox regression model. Result:82.4% of patients had excellent final results, and 17. 6% of patients were recurred of tic during the follow-up period. The venous compression of the trigeminal root predicted a higher rate of recurrence (P<0.05). Conclusion: Microvascular decompression is an effective treatment for trigeminal neuralgia. It is neccessary to know various influential prognostic factors and to chose proper therapeutic procedure was emphasized.
