青年大肠癌临床病理学特征研究（附136例报告）

目的 探讨青年大肠癌临床病理学特征。方法 回顾性分析四川大学华西医院普外科2005年3月-2007年2月收治的136例40岁以下青年大肠癌患者临床资料。结果136例青年大肠癌患者平均年龄32．28岁，其中30岁及其以下者32例，占23．53％。男女比例1．13：1。136例共检测出病灶141个，其中单病灶132例（97.06％），多发病灶4例（2．94％）。组织学分型为：管状腺癌102例（75．00％），乳头状腺癌2例（1.47％），黏液腺癌26例（19．12％），印戒细胞癌4例（2．94％），类癌2例（1．47％）。Dukes分期：A期10例（7．35％），B期35例（25．74％），C期89例（65．44％），D期2例（1．47％），其中B、C、D期共126例，占92．65％。结论 青年大肠癌表现为病理分化程度低、Dukes分期晚，更具进展性。

The Clinicopathologic Characteristics in Young Patients with Colorectal Cancer: A report of 136 cases

Method Retrospective analysis were made for 136 patients age 40 years or younger who received treatment for CRC at west china hospital from Mar 2005 to Feb 2007. Results Average age of 136 cases with CRC was 32.28. 32 cases among them were younger than 30 years old （23.53%）. The ratio between male and female was 1.13 ： 1. There were altogether 141 CRC lesions found in all the cases. Among them 132 （97.06% ）cases presented with a single lesson, 4（2.94%） eases presented with multipre lesions. Histological types in all the lesions eases were grouped as follows： tubular adenocarinoma 102 （75. 00%）; papillary adenoearcinoma 2 （1. 47%）; mucinous carcinoma 26 （19.12%） ; signet ring tumors 4（2.94%） ;carcinoid 2（1.47%）. The cases with confirmed stage A, B, C and D were 10（7.35%）, 35（25.74%）, 89（65.44%） and 2（1.47%）, respectively, according to Dukes＇staging system. The cases with the progressing stages（ B, C, D stages） were 126 （92.65%）. Conclusion CRC in the young population is more aggressive disease, presents with later Dukes＇stage, and has poorer clinicopathologic characteristics.
