旋转平台假体人工膝关节置换在类风湿性关节炎中的临床应用

摘    要:目的 探讨旋转平台假体全膝关节置换治疗类风湿性关节炎的中期临床疗效.方法 2008年6月至2013年1月采用强生公司生产旋转平台假体(PFC-RP Depuy)对62例类风湿性关节炎患者,77膝行人工全膝关节置换术.患者年龄55 ~ 77岁,平均63.2岁,采用KSS膝关节评分系统、髌骨评分、关节活动度及X线评估.结果 所有患者均得到随访,随访时间为3.7~6.2年,平均5.1年,术后KSS评分系统临床评分由术前(52.3±6.7)分提升至术后(89.6±8.3)分,功能评分由术前(30.1±8.4)分提升至术后(91.6±3.9)分,差异有统计学意义;患者膝关节各项评分、关节活动度均得到恢复,关节功能得到改善.X线检查未见假体周围透亮线、骨溶解、假体松动及聚乙烯垫片脱位等并发症.结论 旋转平台假体人工膝关节置换治疗类风湿性关节炎的中期临床效果满意,长期效果仍需进一步观察.

Total knee arthroplasty with rotating-platform knee prosthesis for rheumatic arthritis
Abstract:Objective To investigate the medium-term efficacy of total knee arthroplasty with rotatingplatform knee prosthesis for rheumatic arthritis.Methods From June, 2008 to January, 2013, 62 patients (77 knees) with rheumatoid arthritis were treated with total knee arthroplasty with rotating-platform knee prosthesis.The patients were 55 to 77 years old, with an average of 63.2 years.All the patients were evaluated by, patella score, and X my.And their joint range of motion was evaluated.Results All the patients were followed up 3.7-6.2 (average 5.1) years.The score of KSS increased from (52.3±6.7) before the operation to (89.6±8.3) after the operation and function score from (30.1±8.4) to (91.6±3.9), with statistical differences.All the scores of knee joint and the joint range of motion had recovered and the joint function had improved in all the patients.X ray found no transparent line around knee prosthesis, no osteolysis, loose knee prosthesis, dislocation of polyethylene gasket, etc.Conclusions Total knee arthroplasty with rotating-platform knee prosthesis for rheumatic arthritis has good medium-term efficacy, but the long-term efficacy needs further study to find out.
