交锁髓内钉与解剖锁定钢板固定术治疗股骨中下段骨折对膝关节功能的影响

摘    要:目的 比较解剖锁定钢板(LCP)和交锁髓内钉(RIIN)两种手术方法的治疗效果.方法 回顾性分析我院2011年8月至2014年6月收治的82例股骨中下段骨折患者的临床资料,其中,40例患者采用交锁髓内钉内固定术,为RIIN组,另42例患者采用解剖锁定钢板内固定术,记为LCP组,比较两组患者的临床疗效.结果 治疗后,RIIN组患者的优良率为97.50%,明显高于LCP组的83.33%,且RIIN组的住院天数、手术时间、术中出血量以及愈合时间等指标分别为(9.2±1.5)天、(90.8±10.3)min、(350.3 ±45.1) ml、(3.0±1.5)月,与LCP组(10.0±2.1)d、(100.5±20.1) min、(500.2±30.2)ml、(4.5±2.0)]月比较显著较低(P<0.05),两组间差异均具有统计学意义.结论 与解剖锁定钢板相比,交锁髓内钉治疗股骨中下段骨折,其创伤更小,手术切口更短,术后修复更快,且骨折愈合更佳,患者可进行早期功能锻炼,从而有效降低了患者术后膝关节功能障碍,有利于患者的预后恢复,值得临床应用.

Internal fixation with locking compression plate versus retrograde interlocking intramedullary nail for distal femoral fractures
Abstract:Objective To explore the effect of internal fixation with locking compression plate (LCP) versus retrograde interlocking intramedullary nail (RIIN) for distal femoral fractures.Methods 82 patients with distal femoral fractures were included into the study.Among which, 40 cases were internally fixed with RIIN and the other 42 cases with LCE The clinical efficacies of these two groups was compared.Results After the treatment, the excellent rate was significantly higher in the RIIN group than the LCP group (97.50% vs.83.33%);the length of hospital stay, operation time, bleeding volume, and healing time were (9.2 ± 1.5) days, (90.8 ± 10.3) min, (350.3 ± 45.1) ml, and (3.0 ± 1.5) months in the RIIN group and were with the (10 ± 2.1) d, (20.1 ± 100.5) min, (500.2 ± 30.2) ml, and (4.5 ± 2.0) months in the LCP group;there were statistical differences between these two groups (P < 0.05).Conclusions Internal fixation with RIIN for distal femoral fractures is less invasive and has smaller surgical incision, faster postoperative repair and better fracture healing than with LCP.It can effectively reduce postoperative knee function disorders and is helpful for patient's prognosis and worth for clinical application.
