腹腔镜胆囊切除术中应用瑞芬太尼复合丙泊酚静脉麻醉的效果分析

目的 探讨腹腔镜胆囊切除术中应用瑞芬太尼复合丙泊酚静脉麻醉的应用效果.方法 收集我院2013年1月至2015年6月收治的腹腔镜胆囊切除术患者80例进行前瞻性研究且随机分为两组,每组40例,对照组患者给予丙泊酚+阿曲库铵麻醉,实验组患者则给予丙泊酚+瑞芬太尼+阿曲库铵麻醉,比较两组患者麻醉过程中生命体征、临床指标、拔管后意识状态情况、苏醒后不良反应与麻醉效果评价等.结果 实验组患者诱导中、手术中与苏醒后SBP、DBP与HR水平与对照组组间比较差异具有统计学意义(P＜0.01);实验组患者苏醒时间、拔管时间与拔管后5 min意识状态评分组间比较差异具有统计学意义(P＜0.01);实验组患者恶心、呕吐与头疼、头晕发生率明显低于对照组,麻醉效果明显优于对照组,差异具有统计学意义(P＜0.05).结论 腹腔镜胆囊切除术中应用瑞芬太尼复合丙泊酚静脉麻醉的应用效果要优于传统丙泊酚+阿曲库铵麻醉,安全性较好,具有借鉴性.

Application analysis of intravenous anesthesia with remifentanil and propofol in laparoscopic cholecystectomy

Objective To investigate the application effect of intravenous anesthesia with remifentanil and propofol in laparoscopic cholecystectomy.Methods 80 patients for laparoscopic cholecystectomy with intravenous anesthesia admitted in our hospital from January 2013 to June 2015 were selected for prospective study, and randomly divided into two groups, 40 cases in each group.Patients in control group were given propofol and cisatracurium anesthesia, while patients in experimental group were given propofol, remifentanil and atracurium anesthesia.Vital signs and clinical indexes during anesthesia, conscious state after pulling tubes, adverse reactions after waking up, estimation of the anesthetic effect of two groups were compared.Results There were statistically significant differences in the levels of SBP, DBP and HR during anesthesia induction, in the course of operation and after waking up between two groups (P＜0.01).There were statistically significant differences in awakening time, extubation time, the score of conscious state at 5 minutes after extubation between two groups (P＜0.01).The incidences of nausea, emesis, headache and dizziness in experimental group were significantly lower than those in control group, the anesthetic effect in experimental group was significantly better than that in control group, with statistically significant differences between two groups (P＜0.05).Conclusion The application effect of intravenous anesthesia with remifentanil and propofol in laparoscopic cholecystectomy is better than traditional propofol and cisatracurium anesthesia, the anesthesia regimen is safe and of reference.
