幕上星形细胞瘤生存分析

目的:分析影响幕上星形细胞瘤患者生存的因素. 方法:选取性别、年龄、部位、术前KPS评分、术前癫痫、病理级别、瘤周水肿、瘤-脑组织界面、肿瘤内钙化、肿瘤囊变、肿瘤家族史、手术方式、术后放疗、肿瘤大小等14个可能影响幕上星形细胞瘤患者生存期的因素用Kaplan-Meier法和Cox回归分析. 结果:用Kaplan-Meier法单因素分析显示年龄、部位、术前KPS评分、病理级别、瘤周水肿、瘤-脑组织界面、肿瘤内钙化、肿瘤囊变、肿瘤家族史、手术方式、术后放疗、肿瘤大小等12个因素是影响幕上星形细胞瘤患者生存期的危险因素;Cox回归多因素分析显示部位、术前KPS评分、病理级别、手术方式、术后放疗、肿瘤大小6个因素对幕上星形细胞瘤患者生存期的影响具有统计学意义. 结论:结合预后影响因素、注意治疗中的各环节,合理安排治疗模式,将有助于不断改善幕上星形细胞瘤患者的预后.

Survival analysis of supratentorial astrocytomas

AIM: To evaluate the factors that affect the survival of the supratentorial astrocytomas patients. METHODS: Kaplan Meier and Cox regression model were used to evaluate the possible prognostic factors: sex, age, tumor locus, KPS score before operation, epilepsy before operation, histological grade, peripheral edema, tumor brain interface status, intratumor calcification, cystic changes of tumor, family history of tumor, type of surgery, postoperative radiotherapy and size of tumor. RESULTS: In analysis of Kaplan Meier, the following characteristics had a significant influence on the survival time: age, tumor locus, KPS score before operation, histological grade, peripheral edema, tumor brain interface status, intratumor calcification, cystic changes of tumor, family history of tumor, type of surgery, postoperative radiotherapy and size of tumor. In multivariate analysis of Cox regression model, the following characteristics had a significant influence on the survival time: position of tumor, KPS score before operation, histological grade, type of surgery and postoperative radiotherapy and size of tumor. CONCLUSION: New instruments and measures will improve the survival of supratentorial astrocytomas patients.
