预防性末端回肠造瘘在低位直肠癌保肛手术中的应用

目的：探讨预防性末端回肠造瘘在低位直肠癌保肛手术中的应用价值。方法：低位直肠癌保肛手术中施行预防性末端回肠造瘘患者，吻合口瘘的发生情况与未施行预防组进行对比观察。结果：施行预防性末端回肠造瘘的56例患者仅3．6％（2例）发生吻合口瘘，但无自觉症状；出现吻合口瘘后的平均住院时间为12天。未施行预防性末端回肠造瘘的76例患者中吻合口瘘发生率11．896（9例），分别予以横结肠造瘘、腹会阴联合切除术及营养支持与局部冲洗引流治疗后愈合；出现吻合口瘘后的平均住院时间为27（15-60）天。结论：在低位直肠癌保肛手术中施行预防性末端回肠造瘘能够有效防治吻合口瘘。

The Application of Prophylactic Terminal Ileum Stoma in the Low-Set Rectal Cancer Protected Anus Operation

Objective： to approach the application value of prophylactic terminal ileum stoma in the low-set rectal cancer protected anus operation. Method： the prophylaxis group and not prophylaxis group has contrast the stoma fistulae happen in the low-set rectal cancer protected anus operation. Results： 2 cases happened in 56 cases suffering the prophylactic terminal ileum stoma patients but not subjective symptom and the average stay was 12 days. 9 cases happened in 76 cases suffering the not prophylactic terminal ileum stoma patients and the patients was concrescence after treating with colon transversum stoma, abdomen-perinaeum association ectomy, nutrition and partly washing. The average stay was 27 days. Conclusion. prophylactic terminal ileum stoma could utility prevention and cure stoma fistulae in the low-set rectal cancer protected anus operation.
