中西医综合治疗慢阻肺合并全身炎症反应综合征81例疗效观察

目的：观察中西医综合治疗慢阻肺（COPD）合并全身炎症反应综合征（SIRS）的疗效。方法：156例接受住院治疗的慢阻肺合并全身炎症反应综合征的患者，随机分为对照组75例和治疗组81例。对照组常规给予控制性氧疗、支气管扩张剂、抗生素、糖皮质激素、痰液净化及液体疗法，必要时强心、利尿、扩血管、营养支持等综合治疗；治疗组在对照组基础上加用补肺清金汤150ml／次，每日3次，温服，10d 1疗程。结果：1疗程后，治疗组在血常规、血黏度、胸片、肺功能及主要症状改善等方面均显著优于对照组（Х^2=7．93，P〈0．005）。结论：补肺清金汤有扶正增免作用，能有效阻断炎症瀑布反应、最大限度的减少细胞的免疫损伤、加强抗生素对病原体的杀灭作用，对于改善慢阻肺合并全身炎症反应综合征病人远期预后，增强运动耐量，提高生活质量均有显著的临床意义。

81 Cases of COPD Complicating SIRS Treated with Chinese Medicine and Medicine Combined Therapy

Objective： to observe the effect of Chinese medicine and medicine on COPD complicating SIRS. Method： 156 cases were divided into control （75 cases） and treatment （81 cases） randomly. The control were treating in oxygen therapy, bronchodilators, antibiotics, glucocorticosteroid, sputum cleaning and fluid therapy or reinforce heart, diureses, expanding blood vessel and nutritional support combined therapy. The treatment added Bufeiqingjin decoction （ 150 ml, tid, po）. The courses of treatment were 10 days. Result： blood routine, Blood viscosity, stemite, pulmonary function and cardinal symptom were better in treatment than control. Conclusion： Bufeiqingjin decoction had the function to regulate immunifaction, block inflammation waterfall reaction, reduce corpuscular immunologic injury and strengthen antibiotics to kill pathogen. It can improve long-term prognosis of patient suffering COPD complicating SIRS and exercise tolerance and quality of life.
