3D时飞法磁共振脑血管造影在缺血性脑血管疾病诊断中的价值(附316例病例分析)

摘    要:目的探讨缺血性脑病的3D时飞法磁共振脑血管造影(three-dimensional time-of-flight cerebra magnetic resonance an-giography,3D TOF CMRA)表现及相关MRI改变。方法对照分析316例各期缺血性脑病的MRI表现,包括30例DSA与MRA对照分析以及22例治疗前后的MRA对比分析,并比较各种三维重建法的显示效果。结果在缺血性脑病的相应供血动脉可有不同程度的狭窄、闭塞改变,血管信号不均匀或部分丢失,侧枝循环形成,脑血管的普遍动脉硬化表现,甚至MRA表现可为阴性。结论MRA和DSA显示血管狭窄程度方面效果基本相似,80%的血管狭窄病灶显示两者相符,血管狭窄或闭塞征象较信号缺如或部分丢失征象可靠;部分急性梗塞病例MRA可表现为阴性;大面积陈旧梗塞有动脉硬化表现及相应血管改变。MRA可部分替代DSA或作为术前筛选检查手段。

Abstract:Objective To study the ischemic cerebrovascular disease manifestations of three-dimensional time-of-flight cerebra magnetic resonance angiography (3D TOF CMRA) and correlations with magnetic resonance imaging. Method Compare analysis the MRI manifestations of each period in 316 ischemic cerebrovascular disease cases, including compare analysis the 30 cases with DSA and MRA, the 22 cases with pretherapy and post-therapy. Furthermore, to compare various kinds the reveal effect of three-dimensional reconstruction. Results There are different grade blood vessel stenosis or occlusion, signal nonuniform or partial lost, circulation formation and cerebrovascular sclerosis appearance of corresponding blood-supply artery in the ischemic cerebrovascular disease. However, MRA manifestation may be negatively. Conclusion MRA may replace digital subtraction angiography (DSA) partially or before the operation being sieve to examine method. The signs are more reliable that the blood vessel of stenosis or occlusion are showed than the lack or partial lost of signal. The MRA manifestation may be negatively in some acute infarct cases but the corresponding blood-supply artery there are usually pathological changes of cerebrovascular sclerosis appearance in large extent and olden infarction .
