单纯后路钉棒系统固定结合PVP术治疗胸腰椎骨转移癌临床疗效报告

目的探讨单纯后路钉棒系统固定结合PVP术治疗胸腰椎骨转移癌的效果。方法对19例出现神经功能受损的患者胸腰椎骨转移癌,采用单纯后路减压椎弓根钉棒系统固定结合骨水泥充填椎体成形术治疗。结果 19例均获得脊椎稳定,术后患者的疼痛、食欲、睡眠获得显著改善,与术前比较差异显著(P0.01),且未随着时间的推移而明显下降,术后2周、1月、3月间比较无统计学差异(P0.05)。术后1、3月分别有63.2%和78.9%的患者神经功能至少获得Frankel一级以上恢复。结论椎体成形术结合经后路椎弓钉棒系统固定术式相对简单、安全并可有效改善脊柱转移癌患者的生活质量。

The Clinical Report of Vertebroplasty Combined with Posterior Fixation with Pedicle Screws for the Treatment of Metastases in the Thoracolumbar Spine

Objective To assess the effect of vertebroplasty combined with posterior fixation with pedicle screws for the treatment of metastases in the thoracolumbar spine.Methods There were 19 patients with spinal bone metastases and the emergence of neurological dysfunction had accepted the treatments including posterior decompression and pedicle screw system fixation combined with bone cement filling.The postoperative quality of life（including pain,sleep,appetite） and the improvement of neurological recovery were explored.Results In all of the 19 patients,the stability of the spine,the postoperative pain,the appetite and the quality of sleep were significantly improved（comparing with the preoperative indexes,P0.01）.The Frankel level of neurological function got restored at least one level in 63.2% and 78.9% of the patients at 1 and 3 months after operation.Conclusion Vertebroplasty with posterior fixation is relatively simple and safe,and can effectively improve the life quality of the patients suffered from spinal metastases.
