普罗帕酮治疗心律失常的疗效分析

目的 :探讨影响普罗帕酮治疗心律失常疗效的因素。方法 :采用 2 4h动态心电图监测 12 0例(男性 93例 ,女性 2 7例 ;年龄 5 1a±s 11a)心律失常病人单用普罗帕酮 10 0～ 2 0 0mg ,po ,tid× 5～ 7d ,分析心律失常类型、病因、运动及普罗帕酮剂量与疗效的关系。结果 :普罗帕酮对室性与房性心律失常总有效率依次为 76%与 69% (P >0 .0 5 ) ;不同病因 (冠心病、心肌炎、扩张性心肌病等 )总有效率依次为 79% ,71% ,69% (P >0 .0 5 ) ;其中对运动诱发的早搏增多者 ,房性早搏及高危性室性早搏 (RonT)疗效最佳 (P <0 .0 5 ) ;剂量与疗效关系不大 ;有效组心率峰值下降 (P <0 .0 5 )。结论 :普罗帕酮对高危性室性早搏及房性早搏疗效最好 ,用药后心率峰值下降可能是药物有效的机制之一。不同病因、普罗帕酮的不同剂量与疗效关系不大。

Therapeutic effect analysis of propafenone in treating arrhythmia

AIM: To analyse efficacy of propafenone (Pro) in treatment of arrhythmia. METHODS: Before and after administering Pro, the 120 patients (M 93, F 27; age 51 a±s 11 a) with arrhythmia were 24 h monitored by dynamic electrocardiogram. The relation between the causes and type of arrhythmia, exercise and rest, efficacy and dosage of Pro was analysed. RESULTS: The efficacious rates of Pro were 76% for the ventricular arrhythmia, 69% for the atrial arrhythmia (P>0.05). The total efficacy in different causes (coronary heart disease, myocarditis, dilated cardiomyopathy) of arrhythmia was similar (P>0.05). Pro was the most efficacious for exercise induced premature beats, atrial premature beats (APB) and ventricular premature beats (VPB) of high danger (R on T) (P<0.05); after administering the drug, there was a marked drop in the peak of heart rate (HR) in the efficacious group (P<0.05). CONCLUSION: Pro is the most effective durg for high dangerous VPB(R on T) and APB. After admitting the drug, the decrease in the peak of HR my be an effective response. The varying etiologic factor and varying dosage of Pro show little relationship to the effectiveness.
