草酸铂经药盒导管植入系统时辰化疗治疗消化道肿瘤肝转移的效果观察及护理

目的：评价草酸铂经药盒导管植入系统肝动脉时辰化疗治疗消化道肿瘤肝转移效果，探讨科学护理方法。方法：将消化道肿瘤肝转移病人49例，随机分为时辰组25例和常规组24例。两组病人均采用草酸铂＋5-氟尿嘧啶＋亚叶酸钙方案，时辰组病人化疗时间设置为：22：00-10：00持续12h滴注5-氟尿嘧啶；亚叶酸钙与5-氟尿嘧啶同步经外周静脉通过输液泵给药；10：00～16：00通过药盒导管植入系统持续6h滴注草酸铂。常规组正常上班时间经外周静脉给药。期间重点强化时辰化疗相关知识的健康教育和药盒导管植入系统护理。按WHO关于实体瘤近期客观疗效标准和抗癌药物毒副反应的分度标准进行判定和观察记录。结果：时辰组近期客观疗效明显优于常规组（P〈0．01）；常规组病人发生末梢神经炎、静脉炎明显多于时辰组（P〈0．01）；其他毒副反应两组间比较差异无统计学意义（P〉0．05）。结论：草酸铂经药盒导管植入系统时辰化疗治疗消化道肿瘤肝转移具有明显的近期客观疗效，并能减低神经毒性反应和静脉炎的发生，减轻病人负性心理，增强治疗信心，提高了病人的生活质量。

Observention the effect of time chemotherapy with oxaliplatin undergoing port-catheter system therapy dgestive tumors complicated with hepatic metastases and its nursing.

Objective ：To evaluate the effect of time chemotherapy with oxaliplatin undergoing port -catheter system therapy dgestive tumors complicated with hepatic metastases and discuss scientific nursing. Methods ：49 patients with dgestive tumors complicated with hepatic metastases were divided into time group（ n = 25 ）and normal group（ n = 24）. All of the two groups accepted project of LOHP ＋ 5 -FU ＋ CF, the chemotherapy time in time group were setup： （22：00 - 10：00） drip 5 -FU continuance 12 h ;（ 10：00 - 16：00） drip oxaliplatin continuance 6 h undergoing port - catheter system. The normal group using normal time. To inhance helath education of time chemotherapy and nursing of port - catheter system. To determinant and observe by advance impersonality effect standard of solid tumour and graduation standard of bad effect with anticaneers. Results： Advance impersonality effect in time group was better than normal group （ P 〈 0.01 ） ;the occur rate of complication in normal group was higher than time group （ P 〈 0.01 ） ; there was no remarkable different betweet two groups in rest bad effect （P 〉 0.05）. Conclusion：There was remarkable advance impersonality effect of time chemotherapy with oxaliplatin undergoing PCS treatment dgestive tumors complicated with hepatic metastase ,it＇s could loss neurotoxicity reaction and phlebitis,reduce patient＇s negative mental ,enhance therapy confidence and improve the quality of patient＇s life.
