低分子肝素治疗冠心病不稳定性心绞痛36例临床观察

目的 观察低分子量肝素治疗冠心病不稳定性心绞痛对控制心绞痛发作的疗效。方法 将冠心病不稳定性心绞痛72例随机分为对照组和治疗组。对照组常规给予抗缺血治疗和抗血小板治疗,治疗组在对照组治疗的基础上给予低分子量肝素5000μ～7500μ皮下注射,每日两次,连用7～10天。结果 治疗组在减少心绞痛发作次数、缩短发作持续时间、减少发作时硝酸甘油用量、缩短心绞痛缓解和消失的时间经统计学处理,具有极显著差异(P<0.01),并能使心电图缺血性ST—T改变改善(P<0.01)。冠心病急性心血管事件减少。结论 低分子量肝素皮下注射治疗冠心病不稳定性心绞痛对控制心绞痛发作疗效好,能改善心肌缺血,减少急性心血管事件发生,出血并发症少,使用方便而安全。

Clinical observation of 36 cases with low molecular weight heparin to treat unstable angina in the coronary heart disease

Objective To observe the curative effect of low molecular weight heparin in controlling happening of angina in coronary heart disease. Methods Divided 72 cases of unstable angina randomly into two groups: control group treated by common methods and treatment group with the low molecular weight heparin. Control group were treated by anti - ischemia treatment and anti - platelet treatment. At the base of treatment on control group, treatment group were treated with low molecular weight heparin 5000u - 7500u IH,twice one day for 7-10 days constantly. Results treatment group had an obvious difference as control group in reducing angina's attacks times, decreasing the length of attacks, reducing the dosage of nitroglycerin, decreasing the length of angina's relieving and disappearing (P < 0.01). And it can improve the situation of ECG ischemic ST - T (P < 0.01), reduce the happening of acute cardiovascular attacks, few bleeding complication (5/36,13. 8% ). Conclusion Low molecular weight heparin has good curative effect in controlling the attacks of angina, improve the situation of myocardial ischemia, reduce the happening of acute cardiovascular attacks, few bleeding complication, application conveniently and safely.
