利尿合剂联合生脉注射液治疗慢性心力衰竭合并低钠血症56例

目的观察利尿合剂联合生脉注射液治疗慢性心力衰竭合并低钠血症的临床疗效。方法将慢性心力衰竭合并低钠血症112例随机分为2组,对照组56例予常规西医治疗,治疗组56例在对照组基础上予羟乙基淀粉20氯化钠注射液、多巴酚丁胺注射液、门冬氨酸钾镁注射液、呋塞米注射液缓慢静脉滴注,同时予生脉注射液静脉滴注,2组均以5d为1个疗程。观察治疗前后6min步行距离(6MWT)、左室射血分数(LVEF)、B型尿钠肽(BNP)、24h尿量及血清电解质指标。结果 2组治疗后6MWT、LVEF、24h尿量、BNP及血清电解质浓度与本组治疗前比较差异均有统计学意义(P0.05),2组治疗后6MWT、LVEF、24h尿量、血清电解质浓度组间比较差异有统计学意义(P0.05),治疗组优于对照组。结论利尿合剂联合生脉注射液治疗慢性心力衰竭合并低钠血症疗效确切。

Clinical effects of diuretic mixture Combined with Shengmai injection on chronic heart failure with hy ponatremia

Objective To explore the clinical effect of diuretic mixture combined Shengmai injection on the treatment of chronic heart failure with hyponatremia. Methods 112 cases with heart failure and hyponatremia were divided randomly into 2 groups： 56 cases in both treatment group and control group. Control group was given conventional treatment （Heart stimulants, diuretics, vasodilator agents, spironolactone, β-receptor blockers and sodium treatment）. In the treatment group, on the basis of routine treatment, Diuretic Mixture combined Shengmai injection were given. Diuretic Mixture components are Hydroxyethyl Starch 40 Sodium Chloride Injection 250 mL, dobutamine 40 mg, potassium-magnesium aspartate 20 mL, furosemide 40 mg. They were daily injected intravenously 15 drops/min. Shengmai injection 40 mL solved in 5% glucose injection 250 mL intravenous drip daily. Treatment course was 5 days in two groups. All patients were checked with 6-minute walking distance test （6MWT）,left ventricular ejection fraction（LVEF）,brain natriuretic peptide （BNP）, 24 hours urine volume, Serum electrolyte before and after treatment. Results Before treatment patients＇conditions in two groups were similar, parameters including 6MWT, LVEF, BNP, Serum electrolyte, 24-hour urine volume showed no significant difference statistically. After treatment on 6MWT, LVEF, BNP and 24-hour urine volume there were highly significant difference （P〈0.01） between two groups. difference of serum electrolyte concentration in two groups was significant （P〈0.05）. Conclusion Diuretic mixture combined with Shengmai injection can improve the clinical conditions on treatment of chronic heart failure with hyponatremia.
