慢性阻塞性肺疾病患者营养状况的评估及临床意义

摘    要:目的评估慢性阻塞性肺疾病(COPD)患者营养状况,探讨营养不良对COPD的影响。方法对360例COPD患者进行三头肌皮褶厚度、体重、身高、上臂围、上臂肌围、血浆蛋白、淋巴细胞计数检查,计算出体重指数、理想体重比和通常体重比,评估COPD患者营养状况;根据COPD严重度分级标准将所有患者分为4级,探讨营养状况与COPD严重度、死亡率的关系。结果61.67%的COPD患者存在营养不良,随着营养状况的恶化,重症患者逐渐增加,死亡率逐渐增加。结论绝大多数COPD患者存在营养不良;营养不良使COPD病情加重、死亡率增加。

Abstract:Objective To evaluate the nutritional status in COPD patients and investigate the influences of malnutrition to COPD. Methods The triceps skinfold thickness, weight, height, arm circumference, arm muscle circumference, plasma protein and lymphocyte were examined,the body mass index (BMI) and ideal body weight ( % IBW) ,usual body weight ( % UBW) were calculated, and the nutritional status was evaluate in 360 COPD patients; All patients were divided into 4 classes according to COPD severity criteric, and investigated the relationship between nutritional status and COPD severity, mortality. Results 61.67% of COPD patients were with malnutrition; With the deterioration of nutritional status, severe patients and mortality of COPD increased gradually. Conclusion Most COPD patients are malnutrition ; Malnutrition can lead to the conditions exacerbated and the mortality increased in COPD.
