小儿急性白血病的免疫表型分析及其临床意义

摘    要:目的探讨儿童急性白血病的免疫表型特征及其临床意义。方法采用流式细胞仪对66例急性淋巴细胞白血病(ALL)及23例急性髓系白血病(AML)进行免疫表型分析。结果 ALL患儿淋系抗原以CD19阳性率最高,为70.9%,髓系抗原表达以CD13多见。AML患儿髓系抗原以CD33多见,阳性率为69.6%,淋系表达以CD19多见。My+组与My-组及Ly+组Ly-组相比,在发病年龄、性别、形态学亚型、临床体征及CD34表达差异均无统计学意义,CR率差别有统计学意义。CD3+4组、CD7+组与其阴性组相比,ALL及AML患儿完全缓解(CR)率的差别有统计学意义。结论 ALL及AML患儿分别高表达CD19、CD33;部分患儿同时表达髓系、淋系抗原;对于淋系表达的AML可兼顾ALL治疗;CD34及CD7阳性与治疗反应差相关。

Analysis on the immunophenotype of acute leukemia and its clinical significance in children

Abstract:Objective To analyze the immunophenotype of acute leukemia and its clinical significance in children.Methods Immunephenotype was analyzed by flow cytometry in 66 cases with acute lymphoblastic leukemia(ALL) and 23 cases with acute myeloid leukemia(AML).Results In ALL cases,CD19 was the most common lymphoid antigen and the positive rate was high as 70.9%,while CD13 was the most common myeloid antigen.In AML cases,the most common myeloid antigen was CD33 and the positive rate was 69.6%,and CD19 was the most common lymphoid antigen.There were no significant differences in the onset of age,sex,morphological subtype,clinical signs,CD34 expression between My+ ALL group and My-ALL group,Ly+group and Ly-group.According to the complete remission(CR) rate,there were significant differences between CD34+ group and CD34-group,CD7+ group and CD7-group in both ALL and AML cases,as well as between the Ly+ AML and Ly-AML group.Conclusion CD19 and CD33 are highly expressed in the cases of childhood ALL and AML,respectively.Both myeloid and lymphoid antigens are expressed in some cases.CD34 and CD7 are positively correlated with poor treatment response.
