数字减影血管造影在诊治顽固性鼻出血中的应用

目的探讨数字减影血管造影技术在诊治顽固性鼻出血中的应用价值。方法6例顽固性鼻出血患者使用常规方法止血治疗无效后,采用Seldinger技术经皮穿刺法做右侧股动脉穿刺行数字减影血管造影,查明出血部位及责任血管后施行超选择性血管内栓塞治疗。结果6例患者中5例栓塞同侧颌内动脉一次止血治疗成功,另1例经加栓同侧面动脉及对侧颌内动脉而治愈。结论数字减影血管造影是诊断与治疗顽固性鼻出血的一种有效方法。

Application of digital subtraction angiography on diagnosis and therapy of intractable epistaxis

Objective To explore the diagnosis and treatment of intractable epistaxis with the digital subtraction angiography(DSA). Methods Six cases of intractable epistaxis, which couldn't be cured with usual therapy. According to the seldinger's technology, the patients' right arteria femoralis were punctured, DSA were taken to find the bleeding positions and corresponding artery, superselective endovascular embolism(SSEE) were used to stop the bleeding. Results Five cases were successfully cured after embolism homolateral maxillaries internal artery only once. One case was given the homolateral facial artery and heterolateral maxillaries internal artery embolism, his hemorrhage was stopped, too. Conclusion The DSA is a valuable method on diagnosis and therapy of intractable epistaxis.
