不同象限格子样变性发生视网膜脱离的危险性

目的 研究不同部位的格子样变性发生裂孔和视网膜脱离的危险性。方法 选择了4个象限均存在格子样变性而视网膜脱离只发生在一个象限的92个病例进行了分析。结果 由格子样变性引起视网膜脱离的危险性以颞上象限最高，鼻下象限最低。格子样变性发生圆孔的危险性以颞下象限最高，鼻上及鼻下象限最低；发生马蹄形裂孔的危险性以颞上象限最高，鼻上象限最低。由马蹄形裂孔发生视网膜脱离的危险性以鼻上和颞上象限最高；由圆孔发生视网膜脱离的危险性在各象限没有明显差别。结论 格子样变性是引起视网膜裂孔和视网膜脱离的重要危险因素，应根据变性所在象限及引起视网膜裂孔的性质实施预防性手术。

Relative risk for retinal detachment from lattice degeneration in different quadrants

Objective To accurately asess the relative risk values of retinal breaks and retinal detachment(RD)from lattice degeneration(LD)in different quadrants.Methods 92 cases with retinal detachment in one quadrant and LD in all four quadrants were analyzed.The risks of LD and retinal breaks were analyzed by quadrant.Results The risk of developing RD from LD was highest in the superotemporal quadrant(21.6%,P<0.01) and lowest in the inferonasal quadrant(5.1%,P<0.01).The risk of developing a retinal hole from LD was highest in the inferotemporal quadrant(38%,P<0.01) and low in both the superonasal and inferonasal quadrants(16.9%,15.4%,P<0.01).The risk of developing RD from a retinal tear was higher in the superior quadrants,but the risk of developing a retinal hole showed no difference between the quadrants.Conclusions LD is the main risk factor for retinal break and RD.Prophylactic treatment should be provided accordin to LD in different quadrants as well as the quality of retinl breaks.
