新鲜羊膜移植治疗早期重度眼烧伤的临床观察

目的研究新鲜羊膜移植术对早期重度眼烧伤重建眼表的临床效果.方法急性重度眼烧伤24例28眼,14例14眼(Ⅲ度4眼,Ⅳ度10眼)早期(伤后3周内)行新鲜AMT;另14例14眼(Ⅲ度10眼,Ⅳ度4眼)予以药物治疗.结果术后Ⅲ度4眼眼表重建成功;手术组Ⅳ度10眼、对照组14眼均丧失正常眼表结构、发生睑球粘连,3眼角膜穿孔;所有眼视力下降;手术组眼表上皮化时间显著短于对照组(P<0.05,t检验).结论早期新鲜AMT可作为重度眼烧伤后重建眼表的第1步.

A clinical study on unpreserved amniotic membrane transplantation for acute severe eye burns

Objective To investigate the effect of unpreserved amniotic membrane transplantation(AMT)for acute severe eye burns.Methods In 28 eyes of 24 patients with acute severe eye burns, unpreserved AMT were performed on 14 eyes(grade III: 4 eyes, grade IV: 10 eyes)within 3 weeks after injury. The left 14 eyes(grade III: 10 eyes, grade IV: 4 eyes)were treated with routine medical program.Results After AMT, successful ocular surface reconstruction was obtained in 4 eyes with grade III burns.Abnormal ocular surface and symblepharon developed in 10 eyes with grade IV burns in operation group and 14 eyes in control group; corneal perforation occurred in 3 eyes of these eyes.Visual acuity descended in all eyes.Eyes in operation group took shorter time for ocular surface epithelization than those in control group(P<0.05,t test).Conclusion Unpreserved AMT might be an initial intervention for ocular surface reconstruction at early stage of severe eye burns.
