小儿支气管肺炎免疫功能变化的研究

摘    要:目的:观察支气管肺炎患儿T淋巴细胞亚群、免疫球蛋白及补体C3、C4的水平,探讨患儿发生支气管肺炎时免疫功能的变化。方法支气管肺炎患儿58例为观察组,同期儿保门诊经体检为健康正常的小儿共30例为对照组,分别采用流式细胞仪及免疫速率散射比浊法测定两组小儿T淋巴细胞亚群、免疫球蛋白及补体C3、C4的水平,并进行比较分析。结果观察组患儿CD3+、CD4+T淋巴细胞及CD4+/CD8+比值均明显低于对照组(P<0.05),而CD8+T淋巴细胞水平则明显高于对照组(P<0.05);观察组IgA、IgM、IgG及补体C3水平均明显低于对照组(P<0.05),而两组补体C4水平比较,差异无统计学意义(P>0.05)。结论支气管肺炎患儿机体存在明显的免疫功能紊乱,检测免疫功能对诊断及指导治疗小儿支气管肺炎具有重要临床意义。

Abstract:Objective To observe the levels of T lymphocyte subsets ,immunoglobulins and complement C 3,C4 in children with bronchial pneumonia ,and to investigate the changes of immune function in children with bronchial pneumonia . Methods Fifty-eight children with bronchial pneumonia were chosen as observation group ,30 health children who underwent routine health examination in the same period were chosen as control group .The polychromatic flow cytometry and immunoturbidimetry were use to detect the levels of T lymphocyte subsets ,immunoglobulins and complement C3,C4.Results The levels of CD3+,CD4+,and CD4+/CD8+in the observation group were significantly lower than those in the control group (P〈0.05),and the level of CD8+in the observation group was significantly higher than that in the control group(P〈0.05). The levels of IgA,IgM,IgG and complement C 3 in the observation group were significantly lower than those in the control group(P〈0.05).There was no significant difference in the level of complement C 4 between two group(P〉0.05). Conclusion Immune dysfunction exists in children with bronchial pneumonia .Immune function test has important clinical significance for the diagnosis and guidance of treating bronchial pneumonia children .
