腹腔镜卵巢囊肿剔除术对患者卵巢贮备功能及基质血流的影响

目的探讨腹腔镜卵巢囊肿剔除术对患者卵巢储备功能和基质血流的影响。方法选取卵巢囊肿患者51例,随机分为观察组26例和对照组25例,分别采用腹腔镜下卵巢囊肿剔除术和传统开腹卵巢囊肿剔除术。于术前1 d、术后1个月、2个月、3个月,采用彩色多普勒超声测定患者卵巢基质血流收缩期峰速（PSV）、舒张期最低流速（EDV）、收缩期与舒张期末血流速度比值（S/D）、搏动指数（PI）、阻力指数（RI）,同时采用酶联免疫吸附试验测定血清卵泡刺激素（FSH）、雌激素（E2）、促黄体激素（LH）水平。结果两组患者卵巢基质血流的PSV、EDV、S/D、RI及PI比较,差异均有统计学意义（P均〈0.05）,且各项指标随着治疗时间的延长显著变化（P均〈0.05）;PSV、EDV水平术后先减后增,S/D、PI与RI水平先增后减;在变化幅度上,观察组各项指标显著小于对照组（P均〈0.05）。两组患者FSH、E2、LH水平比较,差异均有统计学意义（P均〈0.05）,各项指标随着时间的延长变化显著（P均〈0.05）,E2水平表现为先减后增,FSH及LH水平表现为先增后减;在变化幅度上,观察组各项指标显著小于对照组（P均〈0.05）。结论腹腔镜卵巢囊肿剔除术对患者卵巢基质供血损伤小,利于卵巢储备功能的恢复。

Effects of Laparoscopic Ovarian Cyst Elimination on Ovarian Reserve Function and Stromal Blood Flow

Objective To explore the effects of laparoscopic ovarian cyst elimination on ovarian reserve function and stromal blood flow. Methods Fifty-one cases of ovarian cyst were selected and randomly divided into observation group（ 26 cases） and control group（ 25 cases）. The cases in the observation group were treated with laparoscopic ovarian cyst elimination while in the control group were treated with conventional ovarian cyst elimination. Color Doppler ultrasound was used to measure peak systolic velocity（ PSV）,end diastolic velocity（ EDV）,PSV / EDV（ S / D）,pulsatility index（ PI）and resistance index（ RI） 1 day before and 1 month,2 months,3 months after treatment,while enzyme-linked immunosorbent assay（ ELISA） was used to detect the serum levels of follicle-stimulating hormone（ FSH）,estrogen（ E2） and luteinizing hormone（ LH）. Results There was significant difference in PSV,EDV,S / D,RI and PI between two groups（ all P〈0. 05）,and the change of each index was more significant with the increasing treatment time（ all P〈0. 05）. The levels of PSV and EDV first decreased and then increased after operation,while the levels of S / D,PI and RI first increased and then decreased,the change ranges of all indexes in the observation group were significantly lower than those in the control groups（ all P〈0. 05）. There was significant difference in the levels of FSH,E2and LH between two groups（ all P〈0. 05）,each index varied more significantly with the increasing treatment time（ all P〈0. 05）,the level of E2first decreased and then increased after operation,while the levels of FSH and LH first increased and then decreased,the change ranges of all indexes in the observation group were significantly lower than those in the control group（ all P〈0. 05）. Conclusion The laparoscopic ovarian cyst elimination has little damage on ovarian stromal blood flow,which is of benefit to the recovery of ovarian reserve function.
