摘    要:目的分析院内血液病患者发生败血症的菌群分布特点和耐药性,为临床合理选择抗生素治疗提供依据。方法回顾性分析血液病住院患者发生败血症142例的临床资料。结果发生败血症的血液病患者共142例。31.88%的患者为大肠埃希菌感染,对碳青霉烯类抗生素均敏感。粒细胞减少83.80%,能找到原发感染部位(以呼吸道感染最为多见,占32.48%)83.20%。结论革兰阴性菌(尤其是大肠埃希菌)是导致住院血液病患者发生败血症的最常见致病菌,大肠埃希菌对碳青霉烯类抗生素敏感,在治疗过程中应积极预防和治疗粒细胞减少。

Abstract:Objective To explore the characteristics of flora distribution and drug resistance in hematopathy patients with hematosepsis,and to provide the references for choosing a optimal antibiotic therapy. Methods The clinical characteristics of 142 hematopathy inpatients with hematosepsis were retrospectively analyzed. Results There were 142hematopathy patients with hematosepsis. 31. 88% of the patients with hematosepsis were infected with Escherichia coli( E. coli) which was sensitive to carbapenem antibiotics. 83. 80% of the patients were found with agranulocytosis,and primary infection sites( the most common infection was respiratory tract infection,which accounted for 32. 48%) could be found in 83. 20% of the patients. Conclusion Gram-negative bacillus,especially E. coli,is the most common pathogen of hematopathy inpatients with hematosepsis,and E. coli is sensitive to carbapenem antibiotics. It's necessary to prevent and treat agranulocytosis in the treatment of hematosepsis.
