神经精神狼疮的临床特征及辅助检查分析

目的对神经精神狼疮(NPSLE)患者的临床特征及辅助检查进行总结分析。方法我院2001-2005年住院治疗的626例系统性红斑狼疮患者中确诊为NPSLE患者84例,对其临床表现及血清学、脑脊液、头颅MRI、CT等辅助检查进行回顾性分析。结果84例患者共表现为14种神经精神症状,最常见表现为狼疮样头痛、精神障碍、癫痫、卒中,45%患者同时存在两种及以上神经精神症状,其中44例行MRI或CT检查,有38例异常。结论神经精神狼疮临床表现多样,常见为狼疮样头痛、精神障碍,脑脊液检查及头颅MRI、CT检查对中枢性狼疮诊断的帮助较大。

Characteristic Manifestations and Accessory Examination of Neuropsychiatric Systemic Lupus Erythematosus

Objective To investigate the characteristic manifestations and accessory examination of neuropsychiatric systemic lupus erythematosus(NPSLE) in clinical practice.Methods A total of 84 NPSLE patients were included.Their clinical manifestations and the results of accessory examinations such as serology,cerebra spinal fluid(CSF),head magnetic resonance imaging(MRI) and computer tomography(CT) were retrospectively analyzed.Results There were 14 NPSLE symptoms found in these patients and the most frequent manifestations of the NPSLE included headache,psychiatric disorder,seizure,and stroke;45% patients existed more than two syndromes.The 44 patients received MRI or CT evaluation and 38 showed abnormal.Conclusion Diagnosis of NPSLE is mainly based on its multiform clinical manifestations,and head MRI and CT examination may be helpful.More specific evaluation requires further development.
