P波离散度与老年高血压病并发心房颤动的关系

目的　探讨P波离散度对老年高血压病并发心房颤动的影响。方法　观察 5 0例老年高血压病并发阵发性心房颤动患者 (观察组 )的最大P波时限 (Pmax)、P波离散度 (Pd) ,并与 5 0例老年高血压病无阵发性心房颤动患者 (对照组 )比较。结果　观察组和对照组的Pmax分别为 12 4.3 1± 10 .11ms和 10 6.12± 8.16ms ,Pd分别为 48.98± 7.81ms和 3 5 .69± 5 .0 6ms ,前者均大于后者 (P <0 .0 1)。在观察组 ,Pmax≥ 110ms时 ,预测敏感性为 76% ,特异性为 77.5 % ;Pd≥ 40ms时 ,预测敏感性为 96% ,特异性为90 %。结论　Pd可能是预测老年高血压病并发阵发性心房颤动发生的敏感心电图指标之一。

Relationship between old hypertension with paroxysmal atrial fibrillation and P wave dispersion

Objective To explore the effect of P wave disperstion in old hypertension patients with paroxysmal atrial fibrillation Methods We measured the maximun P wave dutration (Pmax) and P wave dispersion (Pd) from 12-lead surface electrocardiogram of 50 old hypertension patients with history of PAF (group 1) and 50 old hypertension patients without history of PAF (group 2).Results Pmax and Pd were found to be significantly higher in group 1 than that in group 2 (Pmax, 124.31±10.11ms vs 106.12±8.16ms,p<0.01;Pd, 48.98±7.81ms vs 35.69±5.06ms,P<0.01).Pmax≥110ms and Pd≥40ms separated patients from control group with a higher sensitivity(76% and 96%) and specificity (77.5% and 90%).Conclusion Pd are electrocardiographic markers that can be used for the prediction of old hypertensive patients at risk of developing PAF.
