血浆Hcy水平与DM2慢性并发症的关系

摘    要:目的:探讨高Hcy血症与2型糖尿病(DM2)慢性并发症之间的关系。方法:测定180例DM2患者及30例正常对照组血浆Hcy水平。DM2患者分为无并发症、视网膜病变、周围神经病变、肾病、脑梗死及周围大血管病变6组,每组30例,比较各组间Hcy水平。结果:DM2患者的血浆Hcy水平明显高于正常对照组(P〈0.05)。且组间比较结果显示,DM2合并肾病、脑梗死及周围大血管病变组患者的血浆Hcy水平均显著高于无并发症组(P均〈0.05);但DM2合并视网膜病变、周围神经病变组与无并发症组的差异不明显。结论:高Hcy是DM2合并肾、脑血管及周围大血管病变的危险因素之一,血浆Hcy水平的监测对于早期预防及延缓DM2慢性并发症有指导意义。

Abstract:Objective To study the relationship between hyper-homocysteinemia and development of complication in patients with DM2.Methods Plasma Hcy levels were measured with CLIA in ① 30 DM2 patients without complication ② 30 DM2 patients with retinopathy ③ 30 DM2 patients with peripheral neuritis ④ 30 DM2 patients with nephropathy ⑤ 30 DM2 patients with cerebral infarction ⑥ 30 DM2 patients with peripheral vascular diseases and ⑦ 30 controls.Results As a whole,the plasma Hcy levels in the 180 DM2 patients were significantly higher than those in controls(P〈0.05).Among the diabetics,the plasma Hcy levels in patients with nephropathy,cerebral infarction and peripheral vascular diseases were significantly higher than those in diabetic patients without complication(all P〈0.05),but the plasma Hcy levels in diabetic patients with retinopathy and peripheral neuritis were not much different from those in diabetic patients without any complication(P〈0.05).Conclusion Hyper-homocysteinemia is one of the independent risk factor of development of renal,cerebral and peripheral vascular complications in DM2 patients and monitoring of which is clinically useful.
