肝移植术后感染状况的分析

探讨肝移植病人术后感染的发生状况和主要影响因素。方法回顾性分析2002年6月至2003年5月我院开展的35例病人38次肝移植的临床资料，讨论感染的预防、发生和治疗。结果本组29例病人(76.3％)发生细菌感染，早期以肺部感染为主，后期以腹腔和胆道感染为主，感染的菌种主要为革兰氏阴性杆菌和革兰氏阳性球菌。11例肝移植(28.9％)发生真菌感染，均合并有细菌感染，其中3例再移植均发生真菌感染。肝移植术后发生巨细胞病毒感染7例(18.4％)，2例肝移植患者出现HBV的再感染。

STUDY OF INFECTION STATUS AFTER LIVER TRANSPLANTATION

Objective To analyze the incidence and the influencing factors of infection after liver transplantation.Methods From June 2002 to May 2003 the clinical data of 35 patients who underwent 38 liver transplantation were retrospectively analyzed. The prevention, incidence and treatment of infection were discussed. Results Bacterial infection was confirmed in 29 patients (76.3%). Respiratory tract infection occurred in the early stage after surgery, while abdominal infection and biliary tract infection occurred in the late stage. The most common pathogen were gram-negative bacilli and gram-positive cocci. Fungal infection was confirmed in 11 patients (28.9%) whom all suffered bacterial infection, and 3 cases of retransplantation occurred fungal infection. CMV infection occurred in 7 patients (18.4%), and 2 patients reinfected HBV. Conclusion Various infections of bacteria, fungi and virus occurred frequently after liver transplantation. Early diagnosis and treatment is the key point to improve effect of liver transplantation.
