胃癌组织中E-钙粘蛋白表达与浸润转移的关系

目的探讨 E-钙粘蛋白在胃癌中的表达及其与胃癌浸润转移之间的关系.方法应用免疫组织化学 S-P 法研究 87 例胃癌组织中 E-钙粘蛋白的表达.结果正常胃组织及其良性病变中,E-钙粘蛋白都表达在细胞膜上.87 例胃癌标本中 58 例(66.7%,58/87)呈异常表达.E-钙粘蛋白异常表达在弥漫型胃癌(95.8%,23/24)和混合型胃癌(91.7%,11/12)较肠型胃癌(47.1%,24/51)显著增高,P＜0.01;印戒细胞癌(85.7%,12/14)和未分化癌(87.5%,14/16)较腺癌(56.1%,32/57)显著增高,P＜0.01.E-钙粘蛋白的表达与肿瘤分化程度(P＜0.01)、浸润范围(P＜0.01)和淋巴结转移(P＜0.05)有显著相关性.结论 由 E-钙粘蛋白介导的细胞间黏附作用在胃癌发生、发展及转移过程中发挥重要作用,其在癌组织中表达异常是胃癌重要的恶性生物学特征.正常 E-钙粘蛋白表达的丧失可能成为胃癌恶性程度的一种生物学指标.

Relationship Between the Expression of E-cadherin and the Invasion and Metastasis of Gastric Carcinoma

Objectives To evaluate the E cadherin expression pattern in gastric cancer and investigate its relationship to the invasion and metastasis of gastric carcinoma.Methds The expression of E cadherin was evaluated in 87 specimens of gastric cancer by using the immunohistochemistry technique.Results In benign gastric mucosa,E cadherin was expressed uniformly at the cell membrane.Abnormal E cadherin expression was found in 58 (66 7%)cancer pecimens.Aberrant E cadherin expression was found more often in diffuse (95 8%) and mixed type tumors (91 7%) than that in intestinal type tumors(47 1%), P <0 01.A significantly higher proportion of signet ring (85 7%) and undifferentiated tumors(87 5%) showed abnormal E cadherin expression compared with adenocarcinomas with glandular morphology(56 1%), P <0 01.There was a significant correlation between abnormal E cadherin expression and degree of differentiation( P <0 01),the extent of invasion( P <0 01),and the presence of lymph node metastases( P <0 05),respectlvely.Conclusions Intercellular adhesion mediated by the E cadherin plays an important role in morphological changes both in benign and malignant gastric diseases and the abnormality of E cadherin is a significant malignant character of gastric cancer. Loss of normal E cadherin expression may serve as a differentiation marker in gastric cancer.
