16层螺旋CT双期3D-CTA对脑膜瘤术前评估的价值

目的评估16排螺旋CT双期三维血管造影（3D—CTA）显示脑膜瘤与邻近血管关系的能力。方法回顾性分析61病理证实的脑膜瘤术前双期3D-CTA影像资料，与手术结果进行对照，评估3D—CTA显示脑膜瘤与周围脑血管关系的能力。结果以手术为标准，本研究对42例与临近大动脉接触的脑膜瘤CTA．判定为A型（肿瘤未侵蚀血管）28例，24例与术中发现一致（85．7％），CTA判定为B型（肿瘤侵蚀血管）14例，12例与术中发现一致（85．7％）。对28例靠近静脉窦的脑膜瘤，CTA判定有临近静脉窦侵犯的共18例，全部被手术证实；其中14例CTA判断其侵犯程度与手术一致。结论双期3D—CTA提供了脑膜瘤与邻近大血管、静脉窦及骨骼三维空间准确关系；可明确区分肿瘤周围的动静脉成分；有利于手术方案的制定及提高术中处理的可预测性。
