经尿道前列腺电汽化切除术初期并发症的防治(附580例报告)

目的评价经尿道前列腺电汽化切除术(TUVP)中出血、电切综合征的防治经验。方法对580例良性前列腺增生症(BPH)患者行TUVP的临床资料进行分析。比较在膀胱造瘘下150例术前1周服用非那雄胺组和30例未服用非那雄胺组切除腺体重量、平均出血时间和输血情况。结果两组切除腺体重量差异无显著性,平均手术时间和出血情况差异有显著性。结论术前准备应充分个体化,对腺体>50g或估计手术时间>90min者,应常规服用非那雄胺1周,术中行膀胱造瘘引流,能有效减少围术期出血及电切综合征的发生。

Prevention and treatment of early complications following transurethral electrovaporization of prostate (report of 580 cases)

Objective] To evaluate the effect of finasteride in the treatment of perioperative bleeding associated with TUVP and TURS.Methods] The data of 508 patients with BPH was studied based on the treatment of TUVP.Result] During operation,less blood loss was seen in the one-week finasteride treatment before operation.And TURS of less occurrence was seen in the cyxtoxtomy treatment during operation.Conclusions] Adequate preparation should be individual before operation.The one-week finasteride treatment and cyxtoxtomy drainage treatment can efficiently reduce perioperative bleeding and occurrence of TURS for the patients with volume of prostate of >50g or intending operating time of >90min.
