脊椎转移瘤的影像学诊断与鉴别诊断

目的提高脊椎转移瘤影像诊断与鉴别诊断的准确性。方法搜集经病理确诊及临床证实的脊椎转移瘤56例,密切结合临床资料,综合性分析以CT为例的影像学表现。结果56例转移瘤均累及椎体、附件骨性结构,脊柱转移灶累及多个椎体及附件54例,单个椎体2例。溶骨性转移37例,成骨性转移16例,混合性转移3例,出现病理性骨折3例,椎旁形成软组织肿块7例。病灶位于颈段者3例,胸段15例,腰段20例,骶段3例,颈胸段4例,胸、腰段同时受累8例,腰、骶段3例。结论CT是诊断脊柱转移瘤较重要的影像学检查方法。

IMAGING DIAGNOSIS AND DIFFERENTIAL DIAGNOSIS OF VERTEBRAL METASTASES

Objective To improve the accuracy of imaging diagnosis and differential diagnosis of vertebral metastases. Method 56 cases of vertebral metastases having been pathologically diagnosed and clinically confirmed were collected. Closely combined with clinical data, the imaging features they showed, taking CT for an example, were comprehensively analyzed. Result The annex and structure of the bone and the centrum were involved in all the 56 cases of vertebral metastases, 54 cases of Spinal metastases involving in several centra and the annex, 2 cases involving in single centrum. Among them, 37 cases were Osteolytie metastases, 16 Osteoblastic metastases, 3 mixed, 3 pathological fracture, 3 soft tissue masses formed by the spinal column. While the focus of infection, there are 3 cases locating in cervical segments, 15 in the thoracic, 20 in the lumbar, 3 in the sacral, 4 in the cervicothoracic, 8 in the cervicothoracic, and 3 in the lumbosacral. Conclusion CT is a very important examination in the diagnosis of vertebral metastases.
