宫腔镜腹腔镜联合手术治疗未破裂型宫角妊娠10例分析

摘    要:目的探讨宫腹腔镜联合治疗早期未破裂型宫角妊娠的效果。方法回顾性分析我院2003年5月-2008年5月因宫角妊娠行宫腹腔镜手术治疗的10例患者的临床病理资料,并进行随访。结果10例中,4例宫腔镜下诊刮成功,其余6例行腹腔镜下患侧宫角切开取胚术,手术顺利,无1例中转开腹。10例患者手术平均耗时32min(20-72min),平均失血量55ml(20-320ml),无一例输血,平均住院天数2.3天(2-4天),均痊愈出院。定期随访所有患者预后良好,无任何并发症发生,血?-HCG2周内均降到正常,随访至今10例患者中4例已足月分娩。结论宫腹腔镜联合手术有利于宫角妊娠的早期诊断,对于治疗未破裂型宫角妊娠具有安全,微创的效果,临床应用前景好。

Abstract:Objective To investigate the prognosis of laparoscopic and hysteroscopic conservative management of unruptured comual pregnancy.Methods We retrospectively analyzed 10 cases of unruptured cornual pregnancy which received laparoscopic and hysteroscopic conservative management in our hospital from May, 2003 to May, 2008. All these patients were followed.Results Four cases received hysteroseopic curettage successfully. In the other six eases, the uterine eomu was dissected and the ectopic fetus was taken out successfully. The average surgical time, blood loss and hospitalization period was 32 minutes (20-72), 55ml (20-320), and 2.3 days (2-4). There was not any complication happened or blood transfusion needed. All the patients' serum 13-HCG declined to normal in two weeks. In the following, four patients got pregnancy and full-term delivered.Conclusions Endoscopic conservative treatment is proved to be safe and effective for the unruptured comual pregnancy with good prognosis, and fertility could be preserved.
